Notice of Privacy Practices
This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Our duty
Conceptual Healthcare Corporation, its flagship clinic, and its covered subsidiaries are required by law to maintain the privacy of your protected health information (PHI), provide you this notice of our legal duties and privacy practices, and follow the terms of this notice currently in effect.
Permitted uses and disclosures
Treatment. We may use your PHI to provide, coordinate, or manage your healthcare and any related services. This includes sharing PHI with other clinicians involved in your care.
Payment. We may use and disclose PHI so that we and others may bill and receive payment from you, an insurance company, or another third party.
Healthcare operations. We may use PHI for activities necessary to run our practice, including quality assurance, training, accreditation, licensing, and managing our healthcare network.
Disclosures requiring authorization
Most uses and disclosures of psychotherapy notes, marketing communications, and any sale of PHI require your written authorization. You may revoke any authorization at any time by writing to us at privacy@conceptualhealth.com.
Disclosures permitted without authorization
We may disclose PHI without your authorization for: public health activities, victim of abuse/neglect/violence reports, health oversight, judicial and administrative proceedings, law enforcement when required, organ donation, research with IRB approval, threats to safety, specialized government functions, and workers' compensation.
Your rights
Right to inspect and copy. You may inspect and obtain a copy of your PHI in our designated record set. Inside the patient app, this is one tap.
Right to amend. You may request that we amend PHI you believe is incorrect or incomplete. We may deny if the information was not created by us, is not part of the designated record set, or is accurate and complete.
Right to an accounting of disclosures. You may request a list of disclosures we made of your PHI for purposes other than treatment, payment, or healthcare operations. The patient app surfaces this list in real time on the audit log screen.
Right to request restrictions. You may request a restriction on certain uses and disclosures of your PHI.
Right to confidential communications. You may request that we communicate with you in a particular manner or at a particular location.
Right to a paper copy. You may obtain a paper copy of this notice on request.
Right to be notified of a breach. You will be notified of any breach of unsecured PHI within one hour of confirmation.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at privacy@conceptualhealth.com or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
Effective date
This notice is effective April 2026 and remains in effect until we modify it. We reserve the right to change this notice and the new terms will apply to all PHI we maintain. The current notice is always posted at the URL above and inside the patient app.
